For Members with Dental Insurance Plans

The Plan will pay for the portion of your bill that is not covered by other insurance as follows:

• 100% discount for standard fillings
• 50% discount on root canal, crowns, dentures and bridgework
• 40% discount for cosmetic whitening or bonding
• 20% discount on single or double implant
• 35% discount on three or more implants
• 80% discount on emergency treatment (exam, x-ray and palliative treatment)
• 50% discount on emergency treatment leading to extraction/root canal.

• 90% discount on diagnostic and preventive cleanings (two per year)

1) Members understand that this plan "The Plan" is not an insurance plan or program.
2) SHEL Dental, P.C. provides discounts on his services to members of The Plan. Membership fee for an individual membership is $29.95 per month for a term of 12 month.

4) In order to receive discounts under The Plan, members must be in good standing, with nothing in arrears and a valid method of payment on file.
5) The Plan is a contract with obligations to both the Member and SHEL Dental, P.C. It is understood by members that fees will be charged monthly for a one-year period.
6) Payments for The Plan are due in advance or paid monthly through credit card charges. SHEL Dental, P.C. reserves the right to terminate any enrollment or deny eligibility in The Plan for lack of payment. Returned checks, insufficient fund notices on bank drafts or denial by member's credit card company for payment of the periodic program fee is deemed evidence of non-payment by member. There will be a twenty-dollar ($20.00) charge to be reinstated on the program after such denial. If reinstatement for non-payment happens more than once, a thirty-dollar ($30.00) reinstatement fee will apply.
7) Members may cancel their Plan membership at any time upon written notice to SHEL Dental, P.C. and the return of all Plan material and identification cards. Upon cancellation, members become liable for the full value, at regular pre-discounted prices, less amount(s) paid as monthly membership fees.
8) Membership fees are deemed fully earned upon approval of membership. The member fully acknowledges and accepts this written term.
9) If any of the provisions of this agreement shall be held to be invalid or unenforceable for any reason, the remaining provisions shall continue to be valid and enforceable. If a court finds that any provision of this agreement is invalid or unenforceable, but that by limiting such provision it would become valid or enforceable, then such provision shall be deemed to be written, construed and enforced as so limited.
10) This contract covers and supersedes all conversations, statements and agreements, expressed or implied, between the parties, their agents or representatives. The rights and liabilities under this contract shall extend to the successors, executors, administrators and assigns, of such parties, as though they were in case named herein.
11) This Plan does not cover any outside companies work such as any specialist work in other office.
